We need to see a photo in your primary identification aside from your identifying information. You may send it to us by email at toefl@ets.org.
To request a change in the name or date of birth that appears in your TOEFL® online account, fax a copy of your primary identification (ID) document, with your photograph clearly visible, and a copy of this email to +1-610-290-8972 within the next 3 business days. Be sure to include your ETS ID number, registration number and case ID number 160523-006212 on the fax.

Please allow 10 business days after your send your fax for us to review your documentation and update your account. After the 10 days, log in to your account to verify that your information is correct.
